Milko Alexis is a Voodoo Boys Netrunner in Cyberpunk 2077: Phantom Liberty.

Perhaps due to his fixation with netrunning, Milko's physical body is gaunt and sickly. He has featureless white eyes with dark circles around them which remain open even as he runs the Net.

Milko is a Netrunner of some standing within the Voodoo Boys gang; he belongs to the faction of Voodoo Boys that resides in Dogtown and controls part of the Luxor Heights locale. Milko is well known for a specific method of blackmailing in which he threatens to fatally damage the implants and synthetic organs of his victims using a net attack unless they comply with his demands. He then extorts money and favors from highly placed victims such as corporate employees.

According to some of his subordinates, Milko spends more time in the Net than he does in real space.

If V decides to neutralize Milko Alexis:

Milko dies. It is implied that his role in the criminal organization is filled by another netrunner like him.

If V decides to go along with Alan Noël's request:

Milko remains alive while Noël uses him to reach the ringleaders of the blackmail conspiracy whom are then imprisoned. His future fate is unknown.

This talented kid quickly caught the attention of the local Voodoo Boys, who took him under their wing and raised him as a professional netrunner. A moving story? Not really. Gangs aren't charities and their protection comes at a price. In Milko's case, that price is chronic insomnia caused by remorse.
